CareBase

CareBase is the core foundation module that underpins the CareFirst solution. Designed to support the key care cycle processes across all client groups, CareBase offers the following core functionality:

• Case management from point of contact to care planning
• Workflow processes through a care cycle
• Case recording of all assessment information
• Care planning of all services and support for a client
• Resource management of all providers of care

CareBase delivers the functionality to ensure a timely recording and monitoring of client information. Activated through a user friendly integrated desktop, a user can easily access all work outstanding, schedule key care management tasks and monitor existing clients. From one click a user can quickly access key information on a person to assist in planning any intervention.
