Connecting for Health
The drive to put the citizen at the centre of a choice based model of public services remains at the heart of the government's agenda. Joint working is being driven by key adult initiatives such as Connecting for Health (CfH), Personal Demographic Service (PDS) and Common Assessment Framework for Adults (CAF).
Taking the complexity out of information sharing remains a challenge and this is no more evident than in social care and health. Some of the key challenges include data mapping, security, authorization to use data and data disposal.
We are currently working with two local authority lead partners that have been selected for phase 1 of the CAF Demonstrator site programme and one authority that is involved in the PDS.
